My mother lived there for 9 months and she was taken excellent care of. They cared for her like she was their own family. The nurses and CNA's were knowledgeable and caring. I ate lunch there with her almost everyday and I enjoyed their food. I appreciated the clean and inviting environment.

My father was admitted to Coronado Healthcare Center in Pampa, TX on 8/27/25 for skilled rehab after a stroke, and his experience was extremely troubling. There were not enough staff, and timely care was almost nonexistent. My dad would press his call light to go to the bathroom, and by the time an aide arrived, he had either fallen asleep or had an accident. Eventually, he stopped asking because he knew no one would come in time. His eating and drinking declined drastically, and our family became worried he was dehydrated. After discussing this with the DON, I was told his skin test and vitals “did not indicate dehydration,” yet his bloodwork showed dehydration, which made no sense. Because he was increasingly confused and disoriented, I ultimately had to call EMS myself. The hospital confirmed he was dehydrated and gave him fluids. When we visited, we often found him without his teeth, in the same clothes from the previous day, and clearly not being monitored. After raising concerns with the DON, things would briefly improve, then go right back to the same issues. On one occasion, my dad sat in his wheelchair for over 8 hours. I was furious and contacted the DON again, who assured me things would be corrected—but nothing improved long term. Staff repeatedly told us my father was refusing his medications, yet facility records showed that he had taken them. This discrepancy indicates that some nurses were marking medications as administered when they were not, or documenting refusals incorrectly. This practice is extremely dangerous for a post-stroke patient who relies on consistent medications, and it constitutes serious documentation fraud. We tried to move him, but small-town options were extremely limited. He continued to get weaker. A resident even told me that some people only receive a shower every 2–3 weeks. Some of these residents have no one to advocate for them. Sadly, my father continued to decline and eventually passed away. I want families to know that this facility did not provide the care, dignity, or basic attention that residents deserve. Please be cautious when considering Coronado Healthcare Center for your loved one.

I would give 0 stars for Night shift Nurse doesn't wear gloves when wiping patients and doesn't wash hands when leaving room. When mentioned if she's afraid of catching something she shuckled and said no. Then got smart and put one glove on and said I put gloves on are you happy now. No I'm not.. and left poop in floor and another CNA had to come clean up after her. Morning Shift excellent. Kudos to you all..and thank you. 10 stars and rehab therapy has been good.

Is it possible to give 0 stars?! The so called nurse at nights is a joke!!! Your in health ..... meaning wear your gloves and be proper in health are caring are the ones your there to care for!! And when you are called out for not wearing gloves, or washing your hands and you get rude with us. That is super uncalled for!
