Who We Are
The Milton Home, a life plan community, provides assisted living and long-term care [nursing home care]. Robert P. and Clara I. Milton, life-long residents of South Bend, founded the community overlooking the St. Joseph River to serve as a home for the elderly to spend their sunset years. The first residents were accepted in September 1951. Over the years, additional construction allowed for the expansion of services to meet the growing needs of seniors to include long term care. The Milton Home is funded by Medicare, Medicaid, Waiver and Private Pay. The Milton Home is part of South Bend IN based Sterling Healthcare Management, a division of Menora Financial, Skokie, IL. Sterling Healthcare Management owns and manages four communities in Indiana and three in South Bend.
Even though we all want to stay in our homes to age in place, at some point we may recognize we are either not safe at home, living alone is no longer an option or want the relief of chore-free living. Assisted living at The Milton Home can be the solution. Sterling understands today's savvy baby boomers demand a community with a front and center homelike experience, a culture which is authentic and unique and with the healthcare elements of senior living pushed to the background. Aging in place is important. Our services allow a resident to move into Assisted Living and continue to enjoy their lives, which includes access to all therapies, on-site.
After a hospitalization should a resident require short-term rehab, they come right back to their home to recover in one of our two private rooms. Should a resident's health decline, they do not have to move to a different facility, rather simply down the hall. Residents Start Here . At The Milton Home, cues have been taken from the hospitality industry by adding bold visuals, artwork and embracing our building's historical architectural significance, charm, and location. Today the community is undergoing extensive exterior and interior renovations, all carefully planned to ensure the original unique charm, homey feeling and character of the building are maintained just as Robert and Clara Milton would want.

The Milton Home Mission
The Milton Home seeks to preserve the dignity of its residents and provide a safe, secure and comfortable environment in assisted living. Our community also provides short term nursing and rehab which means if our resident is hospitalized, they return home for rehab and are not forced to go to a community with which they are not familiar, nor does that community know our loved one. We do. Sometimes a high level of care is required as we age in place. If you our your loved needs to cross that bridge, as a resident of The Milton Home, they will simply move down the hall to long term care. Same friends, same staff, same food, same fun, same home.
From our humble beginnings to today, these core principles have not changed. The Milton Home serves as a foundation for the next step in life and continues to make a profound impact on its residents and the community. We will never stop improving the level of service we provide to your loved one.
It was on a vacation trip to California in the mid 1920s that Robert P. & Clara I. Milton discovered the concept of a community where seniors could spend their retired years in a pleasant and supportive environment in the care of compassionate professionals. They were inspired to start a similar community in South Bend, Indiana. Their vision, generous donation, and devoted idealism eventually made their dream a reality. On September 14, 1951, The Milton Home, located at 206 E Marion Street, opened its doors as one of the finest of retirement centers in the state.
The Milton Home seeks to maintain its core values, but changes in customs, trends, and technology have not been ignored. As an aging population has grown, so has the need for different levels of care. In 1975 a nursing home was added and in 2005 the facility became Medicare certified, offering skilled nursing services along with rehabilitation therapy.
Over 60 years later, The Milton Home still embodies its creators original vision of quality, comfort, and stability. While things have changed in South Bend, The Milton Home remains the symbol of old time values; representing a part of history and the cultural heritage of the area.

Skilled Nursing Care
Whether the need is short or long term, if additional healthcare support is required for your loved one, our skilled nursing facility is ready. The Milton Home provides nursing care from a dedicated team of professionals, focusing on the special needs of each individual. Our residents receive 24 hour nursing care, which can include partial or total support with activities of daily living, as well as occupational, speech, and physical therapy programs designed to minimize time needed in the facility. Special skin care and other therapeutic interventions are also available.
We offer a wide variety of services to support the individual needs of our residents. If memory care is a concern, we provide programs designed to maintain a high level of cognitive functionality. Speech therapists are available to help residents regain communication skills after a stroke or other debilitating medical event and our physical and occupational therapy staff are here to help your loved one regain the independence they deserve.
