Our centers treat a wide range of patients transitioning from hospital to home with a focus on returning patients to a normal and productive lifestyle. Through the use of specialized medical and rehabilitation technology and equipment, as well as advanced staff training in complex care conditions, the vast majority of our patients return home and back to active lives in a relatively short period of time. In addition, we offer long-term care for patients who can no longer manage at home.
Within our family of services we offer two memory care options. At our Arden Courts communities we provide specialized memory care in a safe and nurturing environment, created exclusively for persons living with Alzheimer's disease and related dementias. Some of our skilled nursing facilities also offer specialized memory care units that are secure and self-contained.
Whether care is desired in the home a patient has lived in for years, a skilled nursing center or an assisted living facility, caregiving is tailored to fit the patient’s and family’s unique needs. Our hospice services integrate holistic principles with palliative disease management, expert pain and symptom control, family education and psychospiritual support for end-of-life care. We also provide the education, counseling and other resources that can help with emotional needs. Care is culturally sensitive, respecting the traditions and heritage that are important to patients and their family members.
We understand that for most people, home is the most comfortable place of all. Especially following a stay in the hospital or a skilled nursing center, having the love and support of family and friends in familiar surroundings can enhance the healing process significantly. Our team of compassionate caregivers provide health care at home, whether that’s the house patients have lived in for years, a temporary family member’s home or some other community setting they call home. All care and services are coordinated and supervised by a registered nurse or therapist, and are focused on enriching patients Lives by helping them restore their independence and once again feel as if they are in control.
Oftentimes, chronically ill patients could benefit from a higher level of consultation, management and involvement than they are receiving in their current setting. Our medical practice division, has developed an advanced disease management program to help meet the advanced medical care needs of our patients with chronic and advanced diseases who require palliative service but who are not quite ready to elect the hospice benefit. Nurse practitioners help coordinate patient treatment, including pain management, emotional health, goal planning and communication, making a difference in the quality of our patients' lives and the lives of their loved ones.
When individuals no longer want the responsibility of running a household, independent, assisted or personal care living may be the right option. We have a committed team of professionals dedicated to providing quality retirement living with genuine care. Recognizing residents Unique needs, we offer a combination of enjoyable activities, delicious and nutritious meals, companionship, security and comfortable upscale living. Many of our facilities are within or on a campus with a skilled nursing and rehabilitation center so residents Health care needs can conveniently be met if necessary following surgery, illness or injury.
