We strive to make every moment count for our residents and patients at Heartland because we believe in celebrating life at all stages.

Short Term or Home.  Our residents and patients come to us for a variety of reasons.  Many are short-term, having left a hospital needing skilled nursing and/or rehabilitative care to get them back home.  Other residents have chosen us as their home with assisted living care, memory care, or independent living.

The Heartland campus is home to a HealthCare Center, Assisted living (RHA), and the Heartland Pointe independent living homes.  Our campus has a dining room, green rolling hills, and recreational amenities.

Assisted Living

Assisted living provides a comforting option for those who have faced challenges living on their own. Our Residential Home for the Aged (Assisted living) offers residents security and support but still gives them the independence they desire. It also provides for a full and rich lifestyle without the burden of daily responsibilities.

From medication supervision to art and music therapy, our Residents receive appropriate care for their level of need, remaining vital and independent to enjoy all Heartland has to offer. You will have a unique experience with an extended family of staff that truly cares about you.

Independent Living

Nestled in the Stones River Valley, with one of Nashville's most scenic views, Heartland Pointe is the perfect option for those seniors who wish to maintain an active, independent lifestyle without the everyday demands of maintaining their homes.

This quiet, safe neighborhood with its picturesque view offers five unique senior-friendly floor plans. Homes are available in two and three-bedroom models and feature high-quality interior finishes including solid oak cabinetry and crown molding, attached garages with electric door openers, private patios, and full-landscaping and lawn care. And for your safety and peace of mind, we also offer a 24-hour emergency call system.

Surrounded by warm, friendly neighbors, residents of Heartland Pointe enjoy a lively schedule of social, recreational, and spiritual programs. The community calendar is full of activities, including book groups, exercise classes, cultural programs, game nights, Bible Study groups, and shopping excursions.

Heartland Senior Community Health Center

The Heartland is home to a post-acute 24-hour HealthCare Center located in the Stones River Valley area of Donelson, providing care for both short-term and long-term patients. We see many individuals who need skilled nursing care and rehabilitation after a stroke, joint replacement surgery, a cardiac procedure, or a serious illness.

We offer a full spectrum of resources to get you back on the road to health. Passionate and skilled team members provide personalized care with dignity and respect to our residents.

HOMECARE

If your physician decides that home health care is right for you, after an injury or illness, NHC Home Care is here for you. Our experienced team of nurses and therapists are dedicated to provide our patients with the knowledge, skills and care they need to recover, take charge of their own health and be able to remain safely at home.

Most people who require these services are recovering from an illness, injury, surgery or are living with chronic medical conditions with acute changes requiring frequent changes in medication or other treatment. Nurses are available on-call to patients 24 hours a day, 7 days a week.

Home Health Care Services, provided by Registered Nurses and Licensed Practical Nurses, include:
