High Note Hospitality

Our Customer Experience Program focuses on delivering the high notes of customer service by creating a full mind, spirit and body hospitality model. Our goal is to ensure patients are treated as if they are at a fine hotel and leave recommending a Symphony Post Acute Network Member Facility to friends and family. Symphony South Shore has a Director of Customer Experience to help our patients and their families get the best out of their stay with us.

Whether you’re recovering from a medical or work-related condition, or recuperating from an orthopedic procedure, you know that illness and injury can make daily living a real challenge. The focus of this program is on achieving the highest level of functional and medical recovery through individually designed rehabilitation care plans.

Transitional & Rehabilitative Services

At Symphony South Shore, we have improved and enhanced transitional and rehabilitative services wholly for the benefit of our guests. Each guest is assigned a “Transitions Coach.” The Transitions Coach manages the medical and logistical needs required to ensure a smooth transition from hospital to home while promoting function, safety, and independence in as short a stay as possible.

It is our goal for our guests to succeed through treatment and return home to an independent and fulfilling life. Our modern, guest-centered facility—staffed by top physicians, nurses, and therapists –offers guests the quickest recoveries and optimal results. The focus of this program is on achieving the highest level of functional and medical recovery through individually designed rehabilitation care plans.

TRANSITIONAL REHABILITATION

Transitional rehabilitation guests are monitored by a physiatrist through a continuum of care. Guests pursue an individualized care plan, designed by a Physician, Rehabilitation Team, and Discharge Coordinator.

As guests progress in the therapy program, their care plan is modified and new goals are added moving them toward a higher level of function and ultimately independence. Prior to discharge, our staff works with each guest to ensure that their activities of daily living can be met and assess the home environment, if needed.

THERAPY

Guests who participate in our individualized therapy programs will find their strength and stamina return through our rigorous, yet productive therapies. The advanced training of Symphony South Shore’s licensed therapists is complemented by some of the best tools in the industry, including cutting-edge equipment and the latest Nautilus-style exercise machines. Guests can expect goal-driven care, and the entire care team works toward our primary goal—giving guests their life back.
Physical Therapy

Cardiac Care

Our cardiac care program promotes optimum recovery and health for those recovering from heart disease and/or cardiac-related surgery. The goal of the program is to limit the physical effects of cardiac illness and improve cardiovascular fitness and overall health. Each of our guests receives a program designed specifically for them, supervised and implemented by a team of cardiac specialists, including physicians, nurses, physical and occupational therapists, and consulting nutritional and psychological professionals.

Pulmonary Rehabilitation

Symphony South Shore has developed a pulmonary program designed to meet the needs of patients with chronic and acute respiratory illnesses. Our post-hospital Pulmonary Rehabilitation Program is tailored to meet each individual’s needs and manages such illnesses as:

The Pulmonary Rehabilitation Program is designed to control symptoms, improve quality of life and return patients to their optimal level of functioning. The highly skilled inter-disciplinary team will work together to create a personalized plan that addresses patients’ needs and monitors their day-to-day progress. The program maximizes patients’ independent functioning and prepares them for a safe return home.

Orthopedic Rehabilitation

Symphony South Shore offers a comprehensive orthopedic rehabilitation program that provides our guests all the therapeutic, clinical and technical resources needed to address a wide range of orthopedic conditions, including:
