MedBridge: Dedicated Short-Term Care
MedBridge is a separate and distinct unit within our skilled nursing center, designed to meet your needs: short-stay rehabilitation patients transitioning from hospital to home. Specialized staff, including registered nurses knowledgeable in post-acute care, provide medical and rehabilitative support, oversee clinical care and are focused on returning you back to your lifestyle. Our skilled nursing centers must meet specific criteria to earn the MedBridge name.
What To Expect During Your Stay
Post-Hospital Care is designed to help you return home or to a lower level of care after a hospital stay. It begins with an interdisciplinary staff including nurses, therapists, therapy assistants, dietitians, social workers and recreation staff—all charged with helping guide your progress, manage your discharge planning and reduce the probability of return to the hospital.
The expertise of the clinical and rehabilitation team can significantly impact your recovery success. From the moment of your admission at MedBridge, you’ll be evaluated by an interdisciplinary team, including a certified rehabilitation and nursing staff trained to meet your special needs.
YOUR TEAM MAY INCLUDE:
Consulting physicians and physiatrist
Physical and occupational therapists
Social worker and discharge planner
Speech and language pathologists
Together, your team will create a regimen tailored just for you. You’ll use leading-edge equipment to aid in your rehabilitation.
Throughout your stay, our team will monitor your care and report to your physician and managed health care plan. For return home, we may perform a home evaluation if needed and arrange for equipment and home care needs.
MedBridge is taking health care to a new level. Every unit has met demanding criteria to carry the MedBridge name, and has demonstrated a consistently high level of performance in short-term, post-acute services.
Average patient stay of less than 30 days
90% of rehab stay patients return to the community
Less than 18% return to the hospital (below industry average)
A customer satisfaction rating of 85% or better
Not all providers have the same experience or results helping you achieve your goals. There is only one way to ensure you’re enjoying exceptional skilled nursing care. The MedBridge pre-registration option allows you to tour the center, meet your treatment team, verify insurance coverage and complete necessary paperwork.
If you are already in the hospital and are looking for post-hospital care, talk to your discharge planner about MedBridge—your bridge from hospital to home.
